What is FlexTherapy℠?
FlexTherapy℠is a customized rehabilitation method that utilizes a variety of stretch and manual therapy techniques to assist the student-athlete in either gaining or regaining the flexibility and mobility that they have lost or has been decreased as a result from injury or postural and muscular imbalances. There are various methods of treatment to help one to regain and/or improve his or her physical function.
The Risks
As FlexTherapy℠ intends to help resolve the problem of myofascial tension that the student-athlete is experiencing, some risks may arise during the treatment, such as discomfort but never pain. If pain is experienced, please inform us IMMEDIATELY! Stretching and twisting may cause some soreness of stiff muscles and joints. This is normal.
Expectations
There are no guaranteed expectations when one undergoes FlexTherapy℠, as this is wholly dependent on the student-athlete and current situation. But when one undergoes any physical therapy program, it is intended that one will be able to return to his or her prior level of functioning or develop a method to continue what was possible to be performed before the injury that is no longer possible after being permanently injured. When going through the program, it is important that the cilent is truthful with what he or she thinks or feels. Proper communication is important for the progress of the client.
Fees
One warm-up session is for about 10-12 minutes, a minimum of 30 minutes before the student-athletes scheduled event and will be $20 per student-athlete. If you have multiple student-athletes needing to be warmed up or assessed and taped for injuries or joint/muscle stability, group fees can be discussed.
KinesioTaping is $10 per student-athlete, limited to one region. Additional regions of the body are $5 each.
Confidentiality
Under the Healthcare Insurance Portability and Accountability Act of 1996 (“HIPPA”), we are required that all medical information of every individual be kept securely and shall not be disclosed to anyone. This allows the patient to have the right on how his or her information and how it shall be used.
The records we acquire from you shall be used for managing care by Joel Tezeno & DeathiiFat Health and Wellness, L.L.C. staff/team members and will be used for reference for payment or reimbursement for services such as billing or collection. We may also use your information for the assessment and improvement of our activities and business operations.
Informed Consent
CONSENT TO TREATMENT: I consent to FlexTherapy, Assisted Stretch Therapy, and/or related services with Joel Tezeno and DeathiiFat Health & Wellness, L.L.C.. I understand and agree that such services may involve bodily contact, including the use of “hands-on” examination and treatment procedures that may be performed in sensitive areas.
TREATMENT OF MINORS: I, as a parent/guardian of a minor receiving FlexTherapy, Stretch Therapy and/or related services do agree and understand that I have been advised to remain on the premises during therapy and/or training sessions, and waive any claim I may have resulting from failure to do so.
LIABILITY: I know and agree that Joel Tezeno and DeathiiFat Health & Wellness, L.L.C. are not responsible for lost or any damage to any of my personal valuables while on the premises and/or during the time of treatment.
WAIVER AND RELEASE: I, hereby release Joel Tezeno and DeathiiFat Health & Wellness, L.L.C. of all liability, claim, damage, or loss of any kind due to my refusal to accept emergency medical services including but not limited to ambulance/urgent care service, physician services, and emergent medical attention.
